Very disappointing. Does not communicate by phone with patients himself after failed procedures and will not change protocol after failure, no effort to reconsider things after failure in hopes of improving outcome for next try. Does not evaluate for immune-medicated issues in ivf. Not thorough. Has a very big ego but unfortunately his current practice/outcomes/care do not appear to merit this. Quick to blame the woman's eggs when failure occurs instead of pondering what HE can do to help the situation. Does not adequately evaluate men beyond their sperm count-- gives no guidance on supplements, life style changes etc for either men or women. Clinic will not allow you to communicate directly with lab embryologists and ? there seems to be some strife between the clinic /Dr Silber's part and the embryology lab which is at the hospital, which can't be good for patient care. Did not feel that he spent adequate time thinking about individual cases, especially for the amount he is being paid. He believes he knows everything about infertility, and is quick to criticize tests/protocols that he does not use or "believe in" instead of keeping an open mind to the ever-changing body of knowledge of the field. Worst was feeling that you are not a partner in your care-- that it's only his way or the highway-- and "his way" is just the cookie cutter, not individualized, not reevaluating approach so if you are a tough or even slightly unusual case it would not be a good clinic. Also seems really strange that he responds and calls people out by name in their reviews if they give him a negative review. Also if you do get pregnant, after the second blood test you are discharged from the clinic-- so essentially left stranded without an IVF doctor because he is not a Ob/gyn-- so if problems come up (miscarriages, d and c, even routine ultrasounds) then you are on your own, scrambling to find a regular non-ivf doctor who will take care of you

Unfortunately our experience with the vasectomy reversal is almost identical to the below negative reviews. Dr. Silber acted as a salesman and our experience was not a positive one. From the beginning we were told false information. After 8 months there is still no sperm seen. We were given a 95% success rate and was told by Dr. Silber that there should be no reason why the sperm would not come back. Dr. Silber made it a sense of urgency to have the reversal done as soon as possible because I was told after an ultrasound that my egg count was low and it would need to be done ASAP. We then scheduled the surgery for 3 months later. We arrived for surgery and were told right before that Dr. Silber would only reconnect one side! We had been told the whole time during consultations that both sides would be reconnected for the best results. On the day of surgery to hear that only one would be reconnected because success rates were the same was surprising but we were already at the hospital so we took Dr. Silber's word for it. After surgery Dr. Silber said the surgery went great and said healing should be fine and we should have no problem seeing sperm. The recovery was terrible! The incision was at least 5" long. We were from out of town so getting a hold of the 24 hour assistant line or the office was almost impossible. After recovery, we went for our 3 month sperm analysis and there was nothing. Though we were disappointed we were told that it should be seen by the 8 month analysis with no problem. We just had our 8 month analysis and still no sperm. We are very disappointed and have talked to the office but the only thing they have said is that it is now probably best to go a different route and that it will probably not come back. All of this and still no word from Dr. Silber. We have reached out for advice /comments from him and have heard nothing. After spending a LOT of money and having the best success rates quoted we are disappointed and left in the same position than before. I will not go back to Dr. Silber for IVF or anything else and do not recommend him for a vasectomy reversal.

Dr. Sherman J. Silber's response
Dear Jeremy and Carolyn,
When we performed your surgery 8 months ago, we told you it can take 12 months or longer for sperm to return. You had a beautiful reconnection and sperm quality was great. You did have a very difficult case. Your vasectomy had removed a lot of the vas, and it required a very large incision. Although there were other failures around the same time, our results actually are fantastic except in those few cases. Statistics are holding up well and unfortunately you fell in the 5% group. Results are as good on one side as both sides and are less painful. If scarring occurs on one side, it shows you are prone to scarring and this would have happened on the other side. Our protocol is to get 95% success with less pain despite difficult surgery. As you must know, other doctors doing vasectomy reversal do not freeze sperm, and we do. Therefore, if your sperm count does not go up in the next 6 months, we will make it up to you with IVF. Our goal is for you to have a baby. We will make it happen. I am dedicated to your success. I am not “a salesman”.

I wasn’t sure if I wanted to post a review about this topic as it is such a personal experience. However, in early 2017 - I desperately scoured the internet in search of recent experiences related to reversals performed by Dr. Silber. I couldn’t find much after 2012. I have come to the conclusion that the majority of couples do not want to talk about something like this publicly - more so when the results are not what you had hoped for.

My husband had a vasectomy reversal 13 months ago. Our 4, 7 and 12 month semen analysis (SA) were all zero. No sperm seen. We are in contact with 3 couples who had their reversals by Dr. Silber relatively close to ours - give or take a few months. All showing no sperm seen thus far and they were also one sided reconnections.

One of the big areas of concern for us was communication. We had placed the $2,000 deposit to schedule surgery over the phone. I called soon after placing the deposit to request a date change and was told by Jan the date could not be changed to any other day or we lose the 2k.. regardless of reason. Keep in mind, we were still 2 months ahead of the surgery date as well. We went ahead with the original surgery date as planned. We had a phone consultation the day before surgery and Dr. Silber was highly optimistic. Vasectomy was right under 6 years old and this would be my husband’s first attempt ever having it reversed. Nothing was ever mentioned during consultation that Dr. Silber only reconstructs one side now and not both. $11,000 we paid not including travel expenses for one side. We didn’t find out about the one side situation until my husband woke up from anesthesia. After arriving home, I called the office to see what happened and why the right side was not attempted. Was informed by Dena that Dr. Silber no longer connects the second side if he feels the first side had a perfect reconnection. Interesting - this was news to us and never mentioned to us prior.

Incision was large - approximately 5 inches. Healed with no complications, no bruising, and very minimal swelling if any at all. My husband was sore for an extended amount of time, but healing process went great. He took two weeks off work and did no lifting of anything over 10lbs (not even a grocery bag) for 4 weeks. Also no ‘releasing’ until 4 weeks post op. Followed all drs orders.

What hurts the most is a false sense of hope. SA after SA we were just told it can take 12 months for sperm to return by Dr. Silber’s office. Was also informed by his office after month 7 SA, (no sperm seen) that Dr. Silber reviewed the notes from surgery and he thought we had a great chance for sperm return. We were also quoted a patency success rate of 95% during our consultation, but he really didn’t see us falling in that 5%. After getting the post-op report - we found out it didn’t appear to be a great chance for success at all. Our vasoepididymostomy (VE) was connected at the head of the epididymis which based on what little knowledge we do have about this sort of thing.. that’s a worst case scenario. That was not mentioned either.

The hospital itself was fantastic. All the staff there were amazing! I really felt my husband was safe in their care, but at the end of the day - opening this door to a vasectomy reversal has been heartbreaking and expensive for an incomplete procedure.

Dr. Sherman J. Silber's response
Dear Francisco and Tabetha,

I was not aware of your concerns and I certainly want the best for you. Our goal remains for you to have your babies; and we can still accomplish that using Francisco’s excellent sperm that we froze at the time of his surgery. As I had counseled you, Francisco’s vasectomy removed an unusually large amount of tissue. Therefore a very large incision was required and this was a difficult case. The reconnection was perfect, and we felt the sperm quality was wonderful. Though it seemed like an impossible case, we had a beautiful reconnection. It is still possible that in 6 months sperm may return to the ejaculate. We didn’t want to put him through such a big operation on both sides. We did freeze good quality sperm for possible IVF. Most programs don’t freeze sperm in case the vasectomy reversal fails. We do, and we will be able to get Tabitha pregnant with this sperm using ICSI.

We have found that if we have a really good connection on one side, if we do the other side, it will increase the amount of pain but not increase the success rate. If he scars during a beautiful reconnection on one side, he will scar on the other side also. So we would never want to do anything to reduce your chance of success. We freeze sperm in case the patient falls into the 5% category that is unsuccessful, and we can’t predict that. I thought this was explained. The surgery went perfect and we have pictures to verify. However, 5% of the time, scarring occurs, and it would occur on the other side as well. So if you’re a patient that scars, our back up procedure of IVF is better than another surgery. As to the head of the epididymis, if we have a good surgery, it doesn’t matter if it is at the head or tail of the epididymis.

I am sorry that you are upset and angry. We are happy to do IVF with his frozen sperm and we will make this easy for you. Be aware of what I’m telling you, we did everything perfectly and no intent to deceive, and we will make it up to you.

Sherman J. Silber M.D.

This Dr. is probably the best IVF Dr. on this planet. I struggled with big Hospitals for years. They seemed to care more for their success rates than they did me. I almost gave up when someone told me about Dr. Sherman Silber. I knew after waiting 3 hours in his office that he was the Dr., he gave me hope, seemed to have nothing to prove to anyone. Told me the truth. Fast forward 7 years later and my two kids are driving me nuts!!!! Thank you Dr. Silber for bringing me these two special people into my life.

Waited an hour in the waiting room that is adorned with images of Dr. Silber before I was told that it would be at least another hour before Dr. Silber would see me. I told the receptionist I could not wait that long, but would reschedule if this was just an 'off' day. She indicated this was a typical day and did not apologize for the inconvenience. If you don't have a job or anywhere else to be, go see Dr. Silber. Otherwise, don't waste your time like I just did.

I could say a million wonderful things about Dr. Silber and his whole team of people, but I will leave it at this: he gave me hope when I had lost all hope, he never made a promise that he didn't keep, and he is the best at what he does. It takes time to get into your first appointment and waiting room time is a bit long, but overall this has been one of the best experiences! Our baby is set to arrive at the beginning of next year and we couldn't be more thrilled.

We wanted to share our appreciation to Dr. Silber, Dr. DeRosa and their staff. We are truly blessed with the arrival of daughter, Avalynn Grace. Families that have gone through the journey of trying to have a child know the significant challenges involved. The emotional and financial requirements demand strength and determination from the families. The process can be full of disappointments, heartache, and even unexperienced doctors. Dr. Silber, Dr. DeRosa, and their caring staff showed us that with experience, determination, optimism, and compassion, we can succeed. We have a beautiful daughter today and are truly thankful to them.

When my sister offered to be a surrogate for us I knew we had to go to the best fertility doctor, Dr. Silber! We had never done IVF but I had heard great reviews and he now has another person giving him more than 5 stars! He and his team are amazing! I simply cannot say enough about all of them. Dr. Silber is extremely professional. He gave us options but recommended what would be best for us and everything worked perfect. His staff was so comforting and I felt as though they held my hand through the whole process, explaining every detail thoroughly. I highly recommend Dr. Silber and his staff to anyone with infertility issues. We are beyond grateful and appreciate him expanding our family.
