Don't see this doctor. He has deceptive business practices. One of them is that you are required to sign a legally binding contract before seeing him. Courts won't enforce this. It's called an adhesion contract. He's betting you don't know your legal rights and you won't bother reading the form before signing it. The office staff can't even explain parts of it. It is written in bad, complicated legalese that left a legal professional scratching their head.
On the form it clearly says consult an attorney before signing. This is in your new patient paperwork. One of the clauses is for binding arbitration. The way the whole contract is worded is weird. No Contract Law attorney would write this thing.
I'm willing to bet he's been sued for malpractice.
