Dr. Chaulk. I just wanted to take a second to say “ thank you” for being the dedicated, thoughtful, and compassionate doctor that you are. You always go above and beyond for me. I feel elated to know you and have you as my doctor. I appreciate your reliable care year after year. Your positivity, helpfulness, and dedication to myself and all your patients really stand out. Dr Chaulk has made wrong referrals, with wait times taking a year sometimes just to be pushed back to him again by the specialist.
Most recently he has sent the wrong dosage to my pharmacy, twice, and also only sent 1 prescription when two different ones were to be sent.
My pharmacy faxed his office and they havnt gotten a response in over 2 weeks. Most doctors reply to these requests usually in a day or two latest as they’re pertaining to peoples meds. Quite important!
There are no in office appointments, not unless you have a telephone appointment first. Even if you know something has to be looked at and you know it has to be in office, you still have to wait 2 to 3 weeks for the telephone appointment and then another 2 to 3 weeks for an in office appointment.
If you ask the staff to bring something to his attention as it’s important such as a med missing or your meds being wrong I don’t believe they actually do this, because you can call every day for weeks and they will say sorry still nothing and that’s it.

Well iv had him all my life and i can honestly say he is gone to dogs as a doctor he will judge you laugh at things your explaining or if your trying talk about stuff He simply make you feel 100% worst then if you never went in. He gives me panic attacks and makes me feel as if I'm worthless and dont matter He thinks everyone who makes a mistake is not worthy of any chance and make you feel like a fool to go to him about your problems all he wants is to push you out the door. He actuly went from a great doctor to terrible ,only for there are not being any other doctor to go to I wouldn't. He is obnoxiously judgy quick to make you feel like idiot he should just give up and dont give me oh it's to much on his plate it's his job to help and all he is doing is making problems worse, terrible doctor. Stuck up worthless doctor

Since COVID-19 he has been terrible. I realize at the height of the pandemic phone call appts were the norm. However, I now understand that family physicians are now suppose to be back to normal or offering their patients in-person or call appts at the patients choice. Not here in this clinic. His staff is frightened to death. Cannot give you an in-person appt without a phone appt first. Asked staff about when he would be back to normal patient in-person appts I was told “When he tells us he’ll see a patient I person”.
If you note my ratings above I gave Dr. Chaulk a five ⭐️ rating for knowledge. Despite how he is currently running his practice it seems that he has become complacent and more dismissive…….easy enough to do by phone, he has been my doctor for more than 30 years. Over that time he has been wonderful, attentive, quick to refer for tests, specialists, etc. I’ve been told by other health professionals that I’m lucky to have such a good GP. Just wondering……has COVID 19 changed my luck………and certainly not for the better.

Dr. Chaulk became our family doctor when we moved to Paradise 4 years ago. I find him very approachable, easy to talk to, takes the time to listen and to explain things. Very pleased to have him as our family doctor.

Excellent family GP Have never had an issue with him in over 20 years. Always helpful and willing to listen. The staff is wonderfull as well

Dr. Chaulk is an amazing family doctor. Never quick to diagnose and takes the time to follow up.
A bit old school in his ways but has amazing bed side manner and his staff are always super friendly!!!
