Her first available appointment was 2 months away. I made the appointment. A month later her staff cancelled my appointment without giving a reason.

After seeing her for over a year, she wrote the wrong diagnosis on a lab referral form. I can only hope she didn't enter it into my actual medical record. This was my final straw. Previously she was repeatedly behind in "appointments" (often over an hour each time). Her bedside manner is typically frigid, she appears barely interested during visits, and she's apparently absent-minded.
