Refused to fill out a medical form..wanted a nurse practioner at another facility to fill it out. The doctor knew it needed to be filled out within a specific amount of time and didn't care. I asked that the form be filled out and he could have checked off the space saying I wasn't clear for surgery....but felt he didn't have to fill out anything at all. I only asked that he answer the 5-6 medical questions honestly. He chose to answer none. This office has only one girl behind the desk and she uses a typewriter...no computer??? When I asked the girl about the form..she told me the doctor already explained that to me and gave me an annoyed look. Obviously I didn't understand and had the right to know why the form was not being filled out.
