Since I have not had any problems concerning my brain in the past, nor have I had any real health problems that would keep me from working in the past (other than arthritis), I really did not know what to expect when seeing a neurologist. I did at least expect professional businesslike paperwork to be done.
On January 16, 2015, I had my first set of episodes. I went home from work but didn't really think anything major was wrong. I went back to work the next week and on January 24th, the episodes happened again. After a trip to the Emergency Room in Elwood Indiana, I contacted my doctors office in Alexandria and they set me up an appointment with Dr Seth Cahn on February 3, 2015. My first meeting with Dr Cahn went well enough and he set me up with an EEG and MRI. Dr. Cahn seems like a good neurologist that is not the problem. I have only been paid short term disability for 3 days (2/7-2/9/2015). Since then, NONE of the office visit notes or dictation have been sent to my short term disability company (Sedgwick Claims Management Services, Inc. PO Box 14028, Lexington, KY 40512 Phone: 800 492 5678). Which means I am not getting any money.

Today (3/18/2015), I am wearing my 48 hour EEG headgear sitting on my home computer wandering if Dr Cahn will ever be able to figure out how to get the information to Sedgwick. I called Sedgwick to confirm again today to find out if they had received any information from Dr Cahn's office and found out that not only had they not received any information, but they call him everyday! Today marks 22 days without any short term disability pay or approximately $2,200 dollars owed me. I am too far behind on bills and have very little food left to be blaming Dr Cahn's office for inability to get dictation done. It is time for something to happen.

Please let me know immediately what you would like me to consider my options.
