What Aaron Kahn wrote on my medical chart did not reflect the conversation he had with me. Very troubling as now my record reflects items that were NEVER discussed or NEVER even mentioned. He did this twice that I have direct knowledge of. Once with me and once with another family member. Glad he is moving on from CORE.
