Avoid this doctor she did a tooth removal and I think cracked my other back tooth in the process just had that cracked tooth removed after a year of grief and pain. RUN the other direction. Find someone else.

She is not knowledgable enough to offer the services she offers. Dr. Pelak and her staff are very unprofessional.

Poor quality work to say the least. She did two crowns which each had gaps and allowed additional decay, both teeth broke. DO NOT HAVE THIS DENTIST DO ANY CROWNS FOR YOU!!!
