To get a copy of your medical records, you need to send a letter to his old business address (below) with your Name, DOB, and where you would like them sent (Home address, Fax #, Dr address, etc). It will be forwarded to the custodian of record. Takes up to 10 business days. I called DE Division of Professional Regulation to get the info. Surprising these sites don't report he is no longer practicing and is supposedly in prison??? Phone disconnected- if anyone knows how to get records, please advise.

Apparently out of business, according to DE medical site his license was revoked. can not get my records. Telephone is disconnected.
