Failed to identify patient by other than first name - didn't use wrist band or even ask for a my last name. Another patient with same first name on next bed also had breast cancer. Failed to answer my question about what was going to happen and injected me with an unknown substance that caused me to go berserk in pre-op before I could repeat my question. Providence Hospital stated that violations of informed consent are routine and their policy is to ignore them. I had a right to know what was going to be done according to the informed consent, had a right to refuse treatment and he violated that with the blessing of the hospital. If he is your anesthesiologist, run the other way. He doesn't follow printed and signed agreements and the hospital doesn't care.
