1) - lack of follow up on the treatment (in ten months he was over three months on vacation with no follow up);2) - lack of comunicaton with other hospitals; 3) - lack of comunicaton with family members; 4) - disconsideration of patient's wishes in administrating overdosed medication; 5) - disconsideration to the numeros patient's complaints about the medication's adverse reactions: he prescribed 75 mg of RISPERDAL CONSTA injectable every two weeks althow the medication's booklet mentions at chapter Dosage and Administration - Recommended Dose and Dosage Adjustment(pg.33): "The maximum dose should not exceed 50 mg RISPERDAL CONSTA every 2 weeks. No additional benefit was observed with disages greater than 50 mg RISPERDAL CONSTA; however, a higher incidence of adverse effects was observed."
