I used Uniform Medical and I don't see it included above. It was covered 100%. I am rating Dr Belcaster and all his staff as very high, as I received terrefic and ontime service from him and all his staff. I saw him for a Colonoscopy and had been in an emergency situation just prior to my appointment. They went out of their way to make certain that I was taken care of with upmost care and made several follow-up phone calls to make certain that I am recovering well. I would recommend them to anyone who asks.
